What to Expect

Prior to the initial visit - You will need to have all forms and prescriptions from your physician, as well as insurance information on hand to present at registration. Please allow 15 minutes prior to your session to register. You may pre-register on-line. If registration is completed on-line you will need to briefly check in to sign paperwork.

Initial evaluation - The therapy team will provide you with a comprehensive evaluation, including gross and fine motor skills, to address all your functional mobility needs. After the initial evaluation, the appropriate treatment plan will be determined.

Regularly scheduled appointments - Our goal is to improve your quality of life through addressing your needs and limitations. We maintain an open environment to make every patient feel comfortable to express their concerns or ask questions. Every program is personalized to each individual for each and every session. Programs can include but are not limited to: manual therapy, soft tissue mobilization, pediatric therapy, aquatic therapy, and home exercise programs.

It is your health and quality of life that matters to us most. We will work with you to fit your appointments with your schedule as appropriately as possible. Outpatient services hours are 8:00 a.m. to 5:00 p.m. Monday thru Friday. Thank you for choosing the SCKMC Rehab Department for your needs.
